Dependable Health Services Position: Physical Therapist (Home Health) Immediate Hire for Talented Part-time or Full-time Physical Therapist PT (Home Health) We are currently offering a $3000 Sign-on BONUS for full-time Physical Therapists. If you choose to be a full-time Physical Therapist here at Dependable Health Services you can make OVER $110,000. per year. We do not limit the number of patients you can see. WANT MORE? The Physical Therapist is responsible for providing physical therapy services to patients according to a physician's plan of care and adherence to all conditions outlined in the Personal Services Agreement. Physical Therapist (Home Health) Essential Functions: Physical Therapist (Home Health) provides physical therapy services to patients; this may include, but is not limited to: Assessing and evaluating therapeutic/rehabilitative/functional status and participation in the development of the total plan of care. Evaluated home environment Assesses for muscle strength, mobility, gait, ROM - a potential for rehab Directing physical therapy treatment Initiates physical therapy program and instructs other personnel and/or family/caregiver members in a certain phase of physical therapy with which they may work with the patient, as well as instructing them on the goals of the physical therapy program for the patient by participating in case conferences. May train patient in the use of a prosthetic device. Identifies patient and family/caregiver needs for the other home health services and refers as necessary. Prepares and submits clinical and progress summaries based on the attainment of goals. Participates in discharge planning for the patient. Prepares and submits a clinical summary based on the attainment of goals as directed by Organizational policy. Provides physical therapy consultation to home families/caregivers when indicated. Provides in-service education programs for nursing personnel as needed. Participate in peer consultation process. Supervises Physical Therapy Assistants according to organizational policy and State regulations. When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two (2) weeks. Physical Therapist (Home Health) Qualifications: Bachelor's or Master's degree in Physical therapy approved by an accredited organization State Licensed to practice as a Physical Therapist, if applicable Minimum of one-year related experience as a Physical Therapist; community/home health care experience is preferred Demonstrates good verbal and written communication; strong organizational skills Possesses and maintains current CPR certification SPANISH SPEAKING IS A PLUS!!
