Role Overview: As a Social Services Coordinator, you will play a crucial role in enhancing the quality of life for our patients. You will assess patients' needs, develop care plans, and work closely with medical staff to provide comprehensive care. If you are passionate about social work and driven to make a difference, we encourage you to apply. Key Responsibilities: Conduct patient assessments to determine their social service needs. Develop and implement care plans in coordination with an interdisciplinary team. Provide counseling and support to patients and their families. Advocate for patient needs within the facility and in the broader community. Maintain detailed patient records and ensure confidentiality. Oversee discharge planning and coordinate with community resources for continued care. Qualifications: Bachelor's degree in Social Work or related field Experience in a Long term care health setting, particularly in social services. Strong understanding of case management and care coordination. Exceptional communication and interpersonal skills. Ability to work compassionately with a diverse patient population.
