Supervises CNA and coordinates care with other health care workers as assigned.Provides comprehensive initial assessments to all clients and on subsequent visits continually assesses Physical, Psychosocial, Environmental and community resources available utilizing the principles of growth and development over the life spanInitiates Plan of Care, re-evaluates the need for care and modifies Plan of Care as ordered / needed.
